Purpose is to provide a therapeutic community based, aftercare service for over 18s which supports individuals to establish independent living skills, address individualised care plans and to work with the team of full-time aftercare support workers in Ashgrove.
To work with the team in the delivery of the œtherapeutic Model of engagement and to implement the model of working through STEM, CBT interventions and trauma informed care initiatives.
To engage with and build a professional trusting relationship with clients of the Aftercare Unit, treating them with dignity and respect in keeping with the values of Daffodil Care Services.
To carry out as directed by the Aftercare Team, any specific one to one interaction, to engage in case management and care planning through effective key working, and to provide a quality, evidence-based service, based on good practice.
To facilitate the journey through their individual aftercare pathway.
To deliver this Aftercare programme in accordance with Daffodil Care Services policies and procedures.
To support the development of individualised care plans based on service provision parameters and client™s needs.
To induct thoroughly and work cohesively where applicable with Volunteers and students, to support the delivery of this service and to be a participant in the induction and ongoing training of new staff, relief staff, graduates, student placements and community employment participants.
